705. Longitudinal Study of Polyomavirus Excretion in Pregnancy

Background:  BK virus (BKV) has emerged as an important cause of morbidity among kidney and hematopoietic stem cell transplant recipients, highlighting the paucity of knowledge regarding the natural history of this ubiquitous virus.  In order to define the natural history of BKV infection in infancy and the significance of pregnancy-related BKV reactivation, we have begun a prospective longitudinal study of pregnant women and their newborn infants. 

Methods: Maternal urine specimens collected during pregnancy were tested by qualitative and quantitative PCR for detection of BKV and JC virus (JCV).

Results: To date, over 130 subjects have been enrolled and over 500 urine specimens have been analyzed.  Herein we report data on 417 specimens from 58 subjects who have each provided 4 or more urine specimens during the course of pregnancy.  In total, 153 (36.7%) of the 417 specimens were positive for BKV and 82 (19.7%) were positive for JCV.  Co-excretion of JCV and BKV was detected in 20 specimens.  In addition, 49 specimens were considered weakly positive due to low-level viruria just below the reliable threshold for virus detection.  Over 90% of subjects had at least one positive specimen for either BKV or JCV.  There was no association between the BKV or JCV viral load in urine or the frequency of polyomavirus excretion with increasing gestational age.  There was no association of polyomavirus shedding with abnormal urinalysis findings.

Conclusion:  This study has documented the frequency of pregnancy-related polyomavirus excretion at a level much higher than that of historical controls and suggests that the immunologic control of BKV persistence is lost early in pregnancy.  Analysis of virologic and immunologic data during the post-partum period and in infancy will be important for understanding the natural history of BKV infection.
